TOR for MDR-TB Clinical Doctor

Background: MDR-TB has been detected in Timor Leste. As per the WHO Global TB report the estimated MDR-TB among new patients (Primary Resistance) is 20%. As application the Green Light Committee (GLC) was developed and approved in 2005 for eighteen (18) patients. But due to prevailing civil unrest and resource constraints the project could not be started till March 2008. On 24th March 2008 MDR-TB management project was formally launched in the country. Drugs for 8 patients arrived in July 2008, funded by UNITAID (GDF). MDR-TB guidelines have been developed and concerned staff sensitized. A DOTS-Plus committee has been formed. NTP has established linkage with IMVS, Adelaide which supports culture and DST.


A DOTS-Plus center has been established in the NGO facility of Klibur Domin in District Liquica as a Public Private Partnerships model. This NGO facility has been identified for in-patient management of MDR-TNB cases for the initial phase of treatment and later if required. NTP provides staff support to this facility.

Job Purpose: To support the implementation of clinical activities related to MDR-TB management based on the national DOTS-Plus guidelines and contribute to capacity development of health staff in MDR-TB management.
The Clinical doctor will work under the direct supervision of Clinical Director of National Hospital Guido Valdares and administration supervision of Director of community Health.


Duties and Responsibilities:

A. Clinical work
1. Assist with the diagnosis and clinical management of MDR-TB cases identified in the country
2. Provide regular clinical management support to the MDR-TB cases admitted in the intensive phase of treatment at the DOTS-Plus facility
3. Provide follow up care to MDR-TB patients on continuation phase of treatment
4. Coordinate with the doctors in the National Hospital for follow-up care including management of adverse reactions to 2nd line drugs (SLD)
5. Advice the DOTS-Plus committee regarding shifting of MDR-TB patients from intensive phase to continuation phase
6. Coordinate with physicians at the Regional Hospitals and DTC for proper management and follow-up of MDR cases on CP
7. Assist the MDR-TB nurses and other concerned staff in proper recording and reporting as per national guidelines
8. Review periodically the status of DOTS-Plus implementation in consultation with DOTS-Plus Committee, NTP manager, Director of National Hospital and Director of National Laboratory and physician at the Klibur Domin
9. Periodically review the laboratory activities and forward the report to the DOTS-Plus Committee and NTP manager.
10. Working closely with NTP and WHO update the MDR-TB guidelines abased on prevailing international recommendations and in-country experiences.
11. Provide monthly update to the Head of CDC Department and detailed six months and annual report.
12. Any others related work as require and as per direction of the supervisor
B.Training
1. Conduct a training needs-assessment to strengthen in-country capacity for diagnosis, management and follow up including treatment adverse drug reactions to SLD
2. Based on the assessment, plan and conduct training for the doctors and nurse in the national and referral hospitals, DOTS-Plus site and others.
C.Qualification and eligibility
1. Candidate must have medical degree from reputed medical school/college with post graduation in pulmonologist and TB
2. 5-7 years of clinical experience in diagnosis and treating TB and MDR-TB cases is essential
3. Work experience in developing country/s and a GLC project will be an added advantage
4. Good communication skill including excellent written and spoken English
5. Knowledge of Tetum or bahasa In donesia would be advantageous
